In other words, it wasn't the first time they had let their visa expire. Renewing the visa is not a difficult process, so it seems they were simply very careless about it. Reminds me of a friend of mine who made a habit of always paying his gas bill late, and was totally outraged when they finally shut off his gas. "They always let me pay late before!" People get lazy, people get complacent, people forget to pay their bills or renew various permits.
