Because they're planning to saw it off.

See the bottles on the right? Alcohol and possibly other drugs to dull the pain and keep him relatively still. He's tied to the table, there's a tourniquet on his left leg (the one that's propped up), in the lower left corner you can see scissors, a knife and a saw (for getting through the bone).

I don't know what movie it's from though.

It was filmed in the 70's but the actual story is set towards the end of the American Civil War.

If you haven't seen The Beguiled folks, you really should. For me at least it's finer than all Eastwood's later stuff, and shows the guy could actually act.
