A lot of the assumptions about medications were incorrect, there were simply 4 different strengths, and they rose as the letters rose. You could not do much bad by administering the medication, but you could stop someone from doing their night action (specifically the exile fircoal case) if you administered medication.
AD started and played the majority of the game as staff town, but as part of his choice to not join the staff mafia, he was given the special condition of needing to eliminate the staff mafia in a specific order.

Though to be fair to town, that's a heck of a lot of night kills, I don't see how it could be possible any other way to win unless scum kills each other every night..
