Actually, it originally refers to the communist countries such as russia, cuba and china.

Thus the first world would be the democrats, second would be the "communists", and the third would be those too poor to actually have a government.

Edit: in case you didn't know, that's why third world is now usually referred to as developing countries. Political correctness...
