Add zero spoils when teammate kicked for missing turns

When a teammate is kicked for missing turns, teammates should not benefit from their spoils. This is especially true for escalating spoils games. Giving the teammates the missing terrs should be enough offset.

Specifics:
In the game example provided, our team had already knocked out one opponent and was close to knocking out a second, but the second target began missing turns when he had 5 cards. Now that he's been kicked out for missing turns, the remaining teammate has 10 cards (his 5 and his deadbeat mate's 5) which is at least 2, and more likely 3, cashes. This means that team will most likely win, and only because the other player on his team deadbeated.

Teammates deadbeating should not be able to turn the game around this drastically using a tactic that is against CC rules (intentional deadbeating).

This will improve the following aspects of the site:
  • Discourage deadbeating as a win tactic
  • Make the game more fair for opponents; while giving the remaining teammate the deadbeater's terrs helps adjust, without totally throwing the game.
